Job DescriptionJob Title: Coordinator Diabetes Self-Management Training Center (DSMTC) Under general supervision, plans, coordinates, and oversees the patient education services of the DSMTC. Provides direct patient assessment and education. Coordinates design and implementation of education care plans/protocols. Plans and implements a range of education activities as appropriate to support maintenance of American Diabetes Association (ADA) recognition status. Collaborates with other team members to provide continuity of care. Performs a variety of administrative functions required to ensure efficient department operation. RD licensed in the state of Texas - CDE or CDE eligible. Minimum 3-5 years experience directly related to the duties and responsibilities specified. Must have experience in program management and diabetes care.
